scalp parasites or what?

Can anyone help please. My partner, a 55 year old male, was bitten by
a tick or white tailed spider about two years ago. Since then he has
had severe stinging pain in his scalp. He had a scalp reduction
operation about 10 years ago and initially thought the pain was to do
with the sutures from this, which he thought had become infected.
However he now believes he has some sort of parasite in his scalp as
he can feel something moving and stinging under the skin. He believes
he saw a white worm coming out of one of the ulcers one night, though
he could only see it using mirrors. He has shaved all his hair off and
used numerous antithelmatic medications, but to no avail. He is also
having problems breathing and says his lungs hurt. Alvomectin, taken
orally, eased the symptoms (including the breathing difficulties) for
about a day, but then they came back. None of the parasitologists have
been able to find anything and he is getting desperate. Does anyone
have any idea what this could be? He hasn't been anywhere tropical in
the past 2 years but does swim frequently in the local river, which
flows through a local rainforest.
