I had a bit of a look around but I don't think you're going to find any
sites offering on-line rehab. Think of the legal implications. However, the
following may be a useful starting point:

Got the above long link by searching for "stroke" 2 above site.

There's much more to be found (if you're interested in a wider series of
links email me and I'll forward the same) but I think you'll be better
returning to professional help. Even if you have no obvious need for rehab
now they may advise a series of "exercises" and stratagies to make sure you
maximise recovery in terms of time and degree. I imagine you have already
received such advice.

Continue with your efforts, sometimes improvement keeps going on for years
and those who make the effort definitely do better than those who don't (by
a long shot sometimes). You have been lucky but don't lean on it. Think very
seriously about your overall health.
