Good days/Bad days: Chronic Fatigue, Concussion, etc.

I am a neuropsychologist with a stronger "physiological" focus than
most neuropsychologists.  Currently I'm studying variability of
symptoms in various disorders: chronic fatigue syndrome, Lyme disease.
fibromyalgia, post-concussion syndrome, MS.

I am asking people who experience changes in severity and/or pattern of
symptoms from day to day to monitor changes and report them to me.
Most useful would be daily ratings over a period of 1-2 weeks or more,
but it is possible that "one-shot" ratings can contribute something.

You can report on more than one symptom, but please rate them
separately: they may or may not parallel each other in their
variability.  My special focus is on "cognitive" symptoms (e.g.
concentration, memory) and on "lethargy" (fatigue, inertia, decreased
drive, etc.), but believe it is important to see how these relate or do
not relate to others (e.g. pain).

For a given day (identified exactly by month and day of month, NOT day
of the week), rate symptom(s) as:
          (1) worse, better, or about the same as usual
      and (2) worse, better, or about the same as day before.

Identify yourself as little or much as you want, but tell me
           (1) age
           (2) sex
           (3) diagnosis
           (4) possibly relevant history (if concussion: more than
               one?  dates of each?)
           (5) geographical location (zip code or phone area code)

RATE THE DAY BEFORE YOU HEAR WHAT OTHERS SAY!  To be useful, ratings
must be independent of possible influence of responses by others.
