8th edition of Bailey & Scott's Diagnostic Microbiology (Ellen Jo Baron and
Sydney M. Finegold) has the formula and procedure in Appendix B, pp. A35-6.
I imagine the 9th edition (published last spring) also has it in the
appendix, but I don't know the pages.
