I would refer you to a draft document prepared by the CDC entitled
"Preventing the spread of vancomycin resistance - a report from the
hospital infection control practices advisory committee", DHH Billing code
4163-18-P. It has been published in the Federal Register. Copies may be
obtained by calling the VRE Report Center at 404/332-2569. This document
contains recommendations for education and use of vancomycin.
