While I too am wondering if the guy is indeed a crackpot, I think you misread
his double entendre. I'll repeat the message with some minor editing and see

I removed the word other and pointed out the period for clarification. I
doubt even the dumbest of MDs don't believe Japan or Hawaii are 'European
countries.'
